Center COVID-19 Screening Questions
During medium or high COVID-19 Hospital Admission Levels as determined by the CDC, for the safety of everyone, all staff, patients, visitors and essential persons must pass this screening process when entering the Center. When Hospital Admission Levels are high, all persons will be required to wear a facemask.
