Co-pays, co-insurance, and deductibles are all part of what is referred to as patient responsibility, meaning the patient is responsible for paying these amounts.
We cannot bill your insurance for these amounts.
We will notify you by phone, and in writing if time allows, of your patient responsibility prior to your procedure.
It is our policy to collect all patient responsibilities (co-pays, co-insurances and deductibles) the day of the procedure. This includes Medicare deductible and 20% co-insurance.

Depending on your insurance, you may be responsible for paying part or all of the cost of your procedure out of pocket. If this is the case, we may be able to provide you with a payment plan. The terms of our payment plans are as follows:
For total balances over $600, we require payment of half of the balance at least one week prior to the procedure date. The remaining balance will be due the day of the procedure.
If you are not able to pay the entire remaining balance the day of the procedure, the balance may be paid over the next 3 months. The first payment will be due the day of the procedure.
For smaller balances, the first half is due the day of the procedure and the remaining balance may be paid over the next 3 months.
In order to set up a payment plan, we require a credit card to be kept securely on file. Your credit card will charged monthly for the agreed upon amount until the balance is paid in full.
Patients who fail to meet the terms of the payment plan will be contacted. If payment is not received, the account will be sent to collections.
